What You Must Bring To Your D.O.T. Physical:
- Your driver's license (Govt. Issued Pictured ID)
- A list of medications you are taking and the name(s) of the doctor(s) who prescribed them.
- Any heart stress tests (actual printed results) if applicable to a heart condition such as Stents, Bypass Surgery, Etc...
- Diabetic/Vision exemptions from the federal government if applicable.
- D.O.T Physicals cost $80.
Syvrud Chiropractic Clinic gets drivers in and out faster than other facilities in our area. Same day appointments are not unusual. Drivers will not have to waste their time waiting for their appointments once they are in our clinic. Turnaround time is usually 30-40 minutes.
